Re-Enrollment Application for Undergraduate Students
Former students wishing to return to 91制片厂 to complete additional course work must submit a Re-Enrollment Application to the Registrar's Office.
1. REQUEST: Please complete the Application for Re-Enrollment (PDF). Type your responses into the fields or use the mouse to select options, then print and sign the form. Adobe庐 Acrobat庐 Reader庐 is required when viewing, completing, and printing PDF files. You can download a free version of this software by
IMPORTANT: To comply with federal regulations governing the privacy of educational records, 91制片厂 requires the penned signature of the student to release academic records. Electronic/digital signatures will not be accepted.
2. UPLOAD: Scan your signed/completed form and upload it using 91制片厂's Secure File Upload Service. to upload your request.
You will receive a notification once your file has been uploaded successfully, as well as an email confirmation to the address you provided. Once completed, please submit official transcripts of any post-secondary coursework since terminating your studies at 91制片厂 to the Registrar's Office. No application fee is required.
